Ü Is in one of the 7 FCGLR states: Illinois, Indiana, Iowa, Michigan, Minnesota, Ohio, and Wisconsin.

Ü 501(c)(3) organization

Ü Volunteer based organization

Ü Less than 25% revenue from 3^rd parties

Ü Clients have no insurance

Full Membership – available to clinics within the FCGLR states

$100 annual dues
